How Do I Build a Form in GoHighLevel?

Short answer

Go to Sites, then Forms, click + Create New Form, add your fields, and save. Then click Integrate to copy the embed code or the direct link. When a new lead submits the form, HighLevel creates a contact for them.

Checked against HighLevel’s help center on . HighLevel updates its interface often, so match the labels you see on screen.

Build the form

  1. 1Go to Sites, then Forms and click “+ Create New Form”
  2. 2Start from a blank form or a template
  3. 3Add fields like name, email, phone, and message, drag them into order, and mark key fields required
  4. 4Use Styles & Options to set layout, colors, buttons, and themes
  5. 5Under Notifications, set an Email Notification for your team and an Auto Responder for the person who submits
  6. 6Choose a confirmation message or a redirect for after submission
  7. 7Save, submit a test entry, and check it on the Submissions tab

Map answers to custom fields

Open Add Object Fields in the builder to add existing fields, or drag in a field type like a dropdown, date picker, or file upload to create a new custom field. Saving the form saves those fields to the account. After that, the field name and unique key are locked in the form and can only be edited under Settings, then Custom Fields.

Keep “score” out of field names

HighLevel treats any custom field with the word “score” in its name as a numeric scoring element in forms, surveys, and quizzes.

Embed it on a website

In the form builder, click Integrate, choose the form type, and click Copy Embed Code. Paste the code into your site’s HTML or code area, then publish and test the page. HighLevel’s guide covers Squarespace, Wix, Shopify, WordPress, and Duda, and each form gets its own embed code.

When a survey fits better

Surveys spread questions across slides. In new or cloned surveys, Conditional Logic v2 adds Jump To. It sends a respondent to another slide based on an answer, so 1 survey can route qualified and unqualified leads down separate paths. Form logic can still show or hide fields, show a message, redirect, or disqualify a lead. Choose a survey when the questions need several steps or answer-based jumps.
